NPTC processes personal data, including sensitive personal information related to psychological and medical history, in accordance with the Data Privacy Act of 2012 (Republic Act No. 10173) and its implementing rules.

2. What We Collect

• Identity & contact information: full name, date of birth, gender, civil status, address, contact number, email.
• Intake information specific to your booking purpose: e.g. employment/education details for employment or school-related assessments; presenting concerns and emergency contacts for clinical or consultation bookings; referring agency, role, and case background for medico-legal evaluations (VAWC, annulment, adoption, criminal case, and similar matters).
• Health-related information: medication history, current medications, prior psychiatric consultation history (where applicable to the service booked).
• Payment information: payment method, reference number, and proof of payment you submit for verification. We do not collect or store full card or bank account numbers.

3. Why We Collect It

• To schedule, conduct, and document your assessment, consultation, or therapy session.
• To route your booking to the appropriate psychologist or psychiatrist.
• To verify payment for services rendered.
• To comply with professional, ethical, and legal record-keeping obligations.
• To send you booking, approval, scheduling, and payment-related notifications.

4. Who We Share It With

Your information is accessed only by NPTC staff directly involved in your case (psychologists, psychiatrists, psychometricians, and administrative staff handling scheduling and billing). We disclose information to third parties (courts, government agencies, employers, schools, law firms) only:

• When you book a service that is, by its nature, intended for that party (e.g. a court-referred evaluation, an employer-requested pre-employment assessment);
• With your explicit authorization; or
• When required by law or a valid court order.

NPTC operates on the FutureOps platform, which provides the underlying booking, scheduling, and case-management infrastructure. FutureOps acts as a data processor and does not use your information for its own purposes.

5. Retention

Clinical records, assessment data, and case files are retained for the period required by applicable professional standards and regulations, and as necessary for the purpose they were collected.

6. Your Rights

Under the Data Privacy Act, you have the right to be informed, to access your personal data, to correct inaccurate data, to object to certain processing, and to request erasure or blocking of your data, subject to legal and professional record-keeping requirements. To exercise these rights, contact the clinic through the details on your booking confirmation.

7. Security

We apply reasonable organizational and technical safeguards to protect your information against unauthorized access, alteration, or disclosure.

8. Changes

We may update this policy from time to time. Continued use of our booking services after an update constitutes acceptance of the revised policy.
